If you are a UCSF employee and have sustained an injury at work, please go to the UCSF Human Resource Department site to learn how to file a Workers' Compensation claim.
If you are a patient or a physician and want to report a medical malpractice related incident, you may contact UCSF Health Risk Management at 415.353.1842.
